    Purpose

    The purpose of this Request for Proposal (RFP) # 58824014 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to create Centers of Excellence that encourage innovation and collaboration among regional health care providers in rural areas. Centers of Excellence are innovative care models that specialize in specific areas of medicine, offer a variety of services and treatments, and create regional access to specialty care with a focus on creating access to specialty care in rural areas where access is currently lacking.

For example, 2 counties in year 1, 3 counties in year 2, 4 counties in year 3?The applicant's proposed service delivery region must include four or more contiguous rural counties or rural portions of counties. The four counties must be included for each year of the project period.
Jun 13, 2023 4:20 PMIf an organization has received a HRSA Planning Grant for FY24, would there be any issues with running these two grants simultaneously in year 1? Both grants would be geared toward the same project, but the funding would go toward different costs (ie, planning costs vs. implementation costs).Grant funds can only be used to cover grant allowable costs and any individual costs must only be claimed from one grant source.

For the deliverable based portion of the funding, what are the reporting requirements after the funding is received?Please refer to the Iowa Department of Public Health General Conditions for audit requirements. After the deliverables have been met and payment has been processed for the deliverables, there are no further reporting requirements.

Applicants must document and describe all formal partnerships that leverage resources to sustain targeted healthcare services. The successful applicant must establish formal agreements for each partnership and submit their roles and responsibilities for the Centers of Excellence program annually by July 31. Question: Is a physician network formal agreement sufficient to meet this expectation of partnership formal agreements? Or are you looking for something different and if so, please specify. Thank you. A physician network is one example of a formal partnership. Successful projects will establish relationships and partnerships with multiple community organizations throughout their service region that leverage resources to sustain targeted healthcare services.
