Excerpts from the RFP:

Purpose

The purpose of this Request for Proposal (RFP) # 58823017 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to plan and implement sustainable policy and systems change(s) to assess patients with prediabetes or patients who are at high risk for developing type 2 diabetes through screening and testing and to increase referrals to diabetes prevention programs (DPP).

Organizations that provide DPP classes have either pending or full recognition by the Centers  for Disease Control and Prevention (CDC) in the Diabetes Prevention Recognition Program  (DPRP). Clinical innovations such as those supported by this RFP are one of the priorities of  the CDC’s 1815: Improving the Health of Americans Through Prevention and Management of  Diabetes and Heart Disease and Stroke CDC-RFA-DP1815.

The Department is actively working to strengthen its capacity to address health inequities in Iowa. Health equity is defined as supporting opportunities for everyone to live the healthiest life possible through the reduction of barriers in environmental, social, economic and other conditions which impact health. This program promotes health equity through working to inform all Iowans on the risks associated with type 2 diabetes, the importance of  being screened and tested, and the opportunity to enroll in a CDC-recognized diabetes prevention program.
