08/12/2020 In the YMCA's Diabetes Prevention Program, how do the funds assist directly with referral of participants into the program? Is the YMCA an eligible applicant for this proposal? Reimbursement for this project will be deliverable-based. The Department has identified the deliverables and corresponding reimbursement amounts. These amounts are all inclusive and no other costs or expenses will be reimbursed. (Section 2.04 - Budget). As long as the applicant is For-profit, not-for-profit, or governmental entity that provides direct patient care, the applicant can be considered eligible for this funding opportunity.
