Purpose
The purpose of this Request for Proposal (RFP) # 58821003 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to provide technical assistance (TA) to promote the adoption and use of electronic health records (EHR) and health information technology (HIT) to improve provider outcomes and patient health outcomes related to identification of individuals with undiagnosed hypertension and high cholesterol and management of adults aged 18-85 with hypertension and high cholesterol.
Clinical innovations such as those supported by this RFP are one of the priorities of the Centers for Disease Control and Prevention’s (CDC) program entitled “Improving the Health of Americans Through Prevention and Management of Diabetes and Heart Disease and Stroke”. In Iowa, this CDC-funded program at the Department is called the “Health Promotion and Chronic Disease Control Partnership” (Partnership).
