Purpose

The purpose of this Request for Proposal (RFP) # 58821005 is to solicit applications that will enable the Iowa Department of Public Health (referred to as the Department) to select the most qualified Applicant to provide the following services:

  1. Iowa Family Support Network

  2. Nine2Thrive Pilot

  3. Children at Home Program

Iowa Family Support Network Program

The Iowa Family Support Network (a statewide coordinated intake, information and referral system) is for families, child healthcare providers, and other professionals seeking information, support and referral for children ages prenatal to eight years. Families, healthcare providers, and other professionals have access to a website and toll free phone number to make referrals and obtain information about the following:

  • Family support (home and group-based) for pregnant women and families with children birth through eight years of age

  • Family support (parent mentor, deaf mentor, or family resource partner) for parents with children birth to eight years of age, who are deaf or hard of hearing

  • Early ACCESS, Iowa’s Individuals with Disability Education Act (IDEA) Part C early intervention program for infants and toddlers (birth to age three) 

  • Newborn hearing rescreens and diagnostic centers

  • Parentivity, a virtual home visitor system

  • Other community supports in Iowa (i.e. resources related to education, health, human services, and insurance)

  • Typical child development

  • Educational and promotional materials for distribution 

Nine2Thrive Program

Nine2Thrive is a public-private partnership bridging primary care and public health services. Nine2Thrive works with expecting mothers to ensure that they have the resources and baby supplies in place for a healthy pregnancy and baby. In addition to providing statewide resource and referral information for families, IFSN will serve as Nine2Thrive information and referral specialists for expecting mothers. Nine2Thrive information and referral specialists will be located in two pilot prenatal clinic sites and provide onsite services. 

Expecting mothers in Nine2Thrive prenatal clinic pilot sites will have access to a Nine2Thrive information and referral specialist in addition to the services offered by the Iowa Family Support Network. Examples of information and referrals provided by the Nine2Thrive specialist include:

  • Child care

  • Safe Sleep

  • Stillbirth prevention

  • Breastfeeding

  • Home Visiting services 

  • Other support needed for a healthy birth outcome, i.e. nutrition, health insurance

Children at Home Program

The Children at Home program is designed to assist families raising a child (less than twenty-two years old) with a disability in obtaining needed services and supports to maintain the child in the home. The implementation of this program includes, but is not limited to reviewing and approving applications, short-term care coordination, ensuring program funds are the payor of last resort, reimbursing individuals and/or businesses for services or supports, and organize and facilitate the Children at Home Advisory Committee.
