Purpose

The purpose of this Request for Proposal (RFP) # 58820027 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to provide technical assistance (TA) for the use of Self-Measured Blood Pressure Monitoring (SMBP) with clinical support among adults ages 18-85 with hypertension.

The Department is actively working to strengthen its capacity to address health inequities in Iowa. Health equity is defined as increasing opportunities for everyone to live the healthiest life possible. This program promotes health equity through increased accessibility and improved quality of care for people with high blood pressure.

01/26/2020 Is this opportunity for counties already receiving assistance through Wisewomen? This funding opportunity is not associated with participation in the WISEWOMAN Program. RFP Section 1.03 defines the Eligible Applicants as nonprofit and for-profit organizations, local boards of health, public health agencies, governmental entities, and health care systems located within at least one of the 18 Iowa target counties (Appanoose, Black Hawk, Cerro Gordo, Clinton, Dallas, Des Moines, Dubuque, Johnson, Linn, Lucas, Mills, Muscatine, Polk, Pottawattamie, Scott, Story, Wapello, and Warren).
