Purpose

The purpose of this Request for Proposal (RFP) # 58820020 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified Applicant(s) providing cardiovascular services to patients of Iowa to receive reimbursement for Care Coordination training and certification for non-physician providers. The Care Coordination training and certification of non-physician providers will assist health systems, public health agencies and other community organizations to implement evidence-based practices to increase high blood pressure control and diabetes and/or high cholesterol management. 


The Department is actively working to strengthen our capacity to address health inequities in Iowa. Health equity is defined as the attainment of the highest possible level of health for all people by achieving the environmental, social, economic and other conditions in which all people have the opportunity to attain their highest possible level of health. This program promotes health equity by engaging qualified applicants to acquire care coordination certification for non-physician providers to implement evidence-based practices to increase high blood pressure control and diabetes and/or high cholesterol management.

11/19/2019 Is there a list of approved training programs that applicants may propose? Can you provide additional clarity surrounding what content must be included in the proposed training program? The selected training program is up to the discretion of the applicant. Applicants should choose the care coordination/health coaching training program they determine will best meet the needs of their organization. The program should provide attendees with adequate training for non-physician providers within their organization to implement evidence-based practices to increase blood pressure control, diabetes management, and/or management of high cholesterol. The training may be completed online, in-person, or a combination of both. Participants must submit proof of program completion and certification prior to June 29, 2020.
