Excerpts from the Application:

 

1.01 Purpose

 

The purpose of this Application Guidance is to provide eligible applicants information on completion of the application for the Childhood Lead Poisoning Prevention Program (CLPPP). Services covered by this application include the coordination of CLPPP activities within their defined jurisdiction.  At a minimum, program activities include medical case management, environmental case management, data management, public education and outreach, and training.

 

1.02 Project and Contract Period

 

Funding during the defined project period is dependent on approval of the Application, contractor performance during the fiscal year, compliance with general and special conditions of the contract, availability of project funds, program modifications, or any other grounds determined by the Department to be in the Department’s best interest. The Department expects the contract period to be a one-year term from July 1, 2019 to June 30, 2020.

 

The issuance of this Application Guidance in no way constitutes a commitment by the Department to award a contract.

 

1.03 Eligible Applicants

 

Applicants must meet each of the following eligibility requirements for consideration.

 

Eligible Applicants

 

Local Boards of Health (LBOH) will be the only entity eligible to apply for and serve as the contractor for the July 1, 2019 - June 30, 2020(FY20) contract. Local Boards of Health (LBOH) have jurisdiction over public health matters in their designated geographic area. A Local Public Health Agency may not apply in response to this posting.

 

Due to limited funding and changes in how the childhood lead program is administered locally, only current Local Boards of Health (LBOH) contracted with the program through June 30, 2019 will be eligible to apply for and receive a  contract for the July 1, 2019 - June 30, 2020 (FY20) contract period.

  • Monona County Board of Health
  • Polk County Board of Health
  • Scott County Board of Health
  • Sioux County Board of Health
  • Siouxland District Board of Health
  • Taylor County Board of Health
  • Warren County Board of Health
  •          Webster County Board of Health

 

 

 

Electronic Communication Requirements

 

Applicant is required to maintain and provide to the Department, upon application, a current and valid email account for electronic communications with the Department.

 

Official email communication from the Department regarding this application will be issued from iowa.grants@webgrantsmail.com. Applicants are required to assure these communications are received and responded to accordingly.

 

1.04 Service Delivery Area

 

Service Delivery area is the county represented by the eligible applicant.

 

Refer to the FY19 Childhood Lead Poisoning Prevention Program Contractors & Service Areas Table posted under the Attachments portion of this Funding Opportunity posting for service counties for each eligible applicant. Any service delivery area changes for this application process must be agreed to by existing contractors and applicable boards of health, and approved by the Department. Submit requests to Kevin Officer at kevin.officer@idph.iowa.gov. Written approval must be obtained prior to submitting an application.  

 

1.05 Funding

 

The source of funding is state appropriated general funds. Funding percentages for each CLPPP program service area is based on a risk model that uses county level data to determine the following variables: (1) percent of pre-1950 housing, (2) percent poverty, (3) population under 6 years in age, (4) percent of children tested, (5) percent of tests 10µg/dL or greater, and (6) percent of confirmed tests 20µg/dL or above. These variables establish the performance and burden portion of the risk model. Each variable is equally weighted and totaled to provide a percent of funding for each of the defined CLPPP program service areas. The Projected Funding Allocation Table is posted under the Attachments portion of this Funding Opportunity posting.

 

Applicants may apply for the allocated amount as designated per jurisdiction for the contract period within the Projected Funding Allocation Table (Attachment F) as posted under the Attachment section of the Funding Opportunity.  

 

Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on the availability of funding or any other grounds determined by the Department to be in the Department’s best interests.
