The issuance of the RFB in no way constitutes a committment by IDPH to award a contract.

Excerpts from the RFB:

 I. INTRODUCTION

 

The purpose of this Request For Bids (RFB) is to solicit bid applications which will enable the Iowa Department of Public Health, hereafter known as the Department, to select the most qualified Applicant(s) providing services to patients within designated target areas of Iowa to receive reimbursement for Care Coordination training and certification for non-physician providers. The Care Coordination training and certification of non-physician providers will assist health systems, public health agencies and other community organizations to implement evidence-based practices to increase high blood pressure control and diabetes and/or high cholesterol management.

 

Project and Contract Period:

 

The project period and contract period shall be from February 1, 2019 to June 29, 2019.

 

Available Funds

 

Applicants may apply for up to $1,750 per non-physician employee to be trained.The total funds available from the Department are $49,000, with an anticipated total of twenty-eight (28) individuals to be trained and certified. Reimbursement funds will not be paid out to the successful applicant(s) until proof of training and certification is received. Training may have started prior to the contract, but training and certification must be completed during the contract period, February 1, 2019 to  June 29, 2019. Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Department’s best interests.

 

The issuance of this RFB in no way constitutes a commitment by the Department to award a contract.

 

 II. SCOPE OF SERVICES

 

The Applicant will assure that the non-physician provider employee(s) participate in care coordination training and meet any requirements necessary for certification during the term of this agreement.

 

  • Training and certification must be completed between February 1, 2019 and no later than June 29, 2019.
  • Proof of certification for each individual must be uploaded in the Progress Reports (Final report) section of IowaGrants by July 15, 2019.
  • Reimbursement will not be paid to the Contractor until the Department verifies certification submission(s).

 

The Contractor shall submit a final report that chronicles the training taken by the non physician provider(s). The report shall be done within the Progress Reports component of the grant site by July 15, 2019.

                                       

 

III. ADMINISTRATIVE MATTERS

 

Eligibility:

 

The following entities are eligible to submit a bid application in accordance with this RFB and must include the applicant’s provision of clinical services within at least one of the following Iowa counties: Appanoose, Black Hawk, Butler, Clinton, Fremont, Lucas, Mills, Montgomery, Muscatine, Page, Taylor and Wapello which are Department determined, CDC approved target area Iowa counties for this project.

 

  • Community Health Centers/Federally Qualified Health Centers (FQHCs) in the designated target counties.
  • Accountable Care Organizations (ACOs) that provide clinical services in the designated target counties.
  • Independent clinical health systems in the designated target counties.
  • Health systems with affiliated Rural Health Clinics in the designated target counties.
  • Health systems with clinics in the designated target counties.
  • Local Public Health Agencies/Local Boards of Health that provide clinical services in the designated target counties.
  • Other community organizations that assist or contract with health systems in the provision of care coordination/health coach services for patients with high blood pressure, prediabetes, diabetes and/or high cholesterol in the designated target counties.

 

Applicants are required to maintain and provide to the Department, upon application, a current and valid email account for electronic communications with the Department.
