It is the responsibility of the Applicant to reveiw all attachments listed in the section below, including the Request for Bid (RFB).

The issuance of the RFB in no way constitutes a committment by IDPH to award a contract. 

Excerpts from the RFB:

 

INTRODUCTION 

The purpose of this Request For Bids (RFB) is to solicit bid applications which will enable the Iowa Department of Public Health, hereafter known as IDPH, to select the most qualified Applicant/s to develop and implement projects and activities for organ donation.

 

Project and Contract Period:

The project period shall be for a nine (9) month term from September 1, 2014 to June 30, 2015.

 

Available Funds

It is anticipated there will be $10,500 available.  Applicants may apply for up to $3,000 for the total nine (9) month period.  Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by IDPH to be in IDPH’s best interests.  A maximum of three (3) applications may be funded.

 

The issuance of this RFB in no way constitutes a commitment by IDPH to award a contract.

 

SCOPE OF SERVICES

Bid Applications should include the Applicant’s plan for accomplishing the work set forth in the below and should include sufficient information regarding the Applicant’s ability to perform the services sought to enable the IDPH to make a judgment about the Applicant’s ability to perform the work identified in the Scope of Services.

 

Consistent with Iowa Code section 142C.15(4)”b”, funding is available in the form of grants to state agencies or nonprofit legal entities with an interest in anatomical gift and transplantation to conduct public awareness projects. 

 

Projects shall target a minority population - Racial/ethnic Iowans (African Americans, Asians and Pacific Islanders, and Hispanics/Latinos) - or individuals aged 55 and older.

 

Funds shall be used for activities promoting public awareness of the anatomical gift program.  Costs of services or materials provided by the Iowa Donor Network (IDN) at 319-665-3787 or the Iowa Lions Eye Bank at 319-356-2871 may not be included as a use for these funds.

 

These funds may not be used for:

    • Direct salary/fringe

    • T-shirts

    • Food

Computer, ipad, ipod, software and/or hardware purchases that exceed a total of $150.

 

ADMINISTRATIVE MATTERS

 

Eligibility

State agencies or nonprofit legal entities physically located in the state of Iowa are eligible to submit a bid application in accordance with this RFB.  Applicant is required to maintain and provide to IDPH, upon application, a current and valid email account for electronic communications with IDPH.
